PRAF 2.0 Ohio Department of Medicaid’s Online Notification of Pregnancy System

Ohio Medicaid Providers/Practices: Select "OHID" from dropdown to log in with your OHID Username and Password to submit pregnancy notifications and referrals for patients currently insured by Ohio Medicaid.

All Other Users, including MCEs, CDJFS and CBOs: Select "Internal" from dropdown to login with your NurtureOhio Username and Password provided to you via email.

What is the PRAF 2.0?

  • ODMs Preferred notification of pregnancy
  • The only pregnancy notification that automatically:
    • Updates Medicaid eligibility in order to:
      • Prevent loss of coverage during pregnancy
      • Prevent payment delays
    • Notifies the managed care plan of pregnancy
    • Assesses for additional patient needs

Who Can Use it?

  • Any Ohio Medicaid contracted individual who identifies pregnancy (OB/GYNs, FQHCs, MCEs, CBOs, PCPs, EDs, ect.)

Updates

Important Update:

On May 22, 2026, NurtureOhio was updated. The following changes were made: 
  • Referrals export will include additional columns:
    • Patient MCO; Referral Status; Archived Referral; Service Provided by MCO; Name of Community Partner; More Information on Referral Outcome; Member received resources; Date Need Created; Date Need Completed; Date Need Last Modified; Date Referral (All Needs) Completed 
  • Data Dictionary changes 
    • Data dictionary is now available under each form Export dropdown 
    • Data dictionary modified to include Referrals export information 
  • PRAF/ROP 
    • Aetna MyCare removed from MCO dropdown selection list 
    • ROP Submitted Date locked at first date of completion
    • ROP Last Modified Date updated with last edited date 
  • User Profile
    • Remove 'Add Practice' functionality 
  • Main site 
    • New accessibility language in footer
  • SFTP/Nightly exports 
    • Institutions receiving nightly files will receive the previous day's submitted and modified PRAFs and ROPs 
 
As of May 1, 2026, the NurtureOhio website has moved to a new domain: https://nurtureohio.medicaid.ohio.gov.
 
Important Update: Multi-factor Authentication for NurtureOhio access begins January 5, 2026 
 Signing into NurtureOhio using State of Ohio ID (OHID) now requires multi-factor authentication (MFA). This added layer of security goes beyond using just your OHID and password by incorporating a second source of validation to verify your identity before granting access to state resources. 
 
For your convenience, here is the MFA user guide that provides detailed instructions on setting up the multi-factor authentication.  If you encounter any issues or require assistance, please contact ODM Integrated Help Desk (IHD) at 800-686-1516, option 7 or email IHD@medicaid.ohio.gov.

    • ​"Primary Language (if not English):" question, now only shows "Primary Language is English?" question is answered "No".
    • "The name of the person at my site who should be contacted with updates/questions about this form is:" question will be required, only when "I would like my patient's Managed care plan to communicate with my office regarding an urgent need." is answered "Yes".
